The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

Mission Neighborhood Health Center’s Clinica Esperanza was established in 1989 to provide culturally appropriate HIV medical services to the Latinx and immigrant community in response to the AIDS pandemic. Clinica Esperanza offers an integrated client-centered medical care approach that includes primary care, medical case management, mental health and substance abuse counseling, and prevention services that include HIV, STI, Hep C testing, PEP, and PrEP to address the needs of patients.

We are seeking a candidate for our Social Services Senior Case Manager & Supervisor position at MNHC's Clinica Esperanza. This position reports to the Director of Programs and HIV Services and is a regular, full Time, exempt role with a salary range of $84,252 to $92,718 with full benefits. 

Primary Objective:
The Social Services Senior Case Manager and Supervisor will be part of an interdisciplinary team and will have supervision of Psychosocial and Care Coordination Services at MNHC’s Clinica Esperanza (CE)

Medical Case Management
Provide direct client support and staff supervision in supporting clients coping with the emotional, psychosocial, and environmental impact that an HIV diagnosis signifies to them and their loved ones with a focus on increasing their quality of life and care through case management, service coordination, access, and primary care delivery to prevent further HIV transmission and reinfection.
The Senior Case Manager and Supervisor has the following responsibilities and duties:

Clinical Responsibilities (60%)

  • Engage CE clients in comprehensive psychosocial assessments addressing medical, mental health, substance use, and other significant social determinants of health.
  • Offer clinical leadership/expertise during psychiatric emergencies, ensuring timely, effective, and ethically sound interventions. Responsibilities include coordinating multidisciplinary responses, conducting risk assessments, maintaining safety protocols, and supporting staff in delivering trauma-informed care.
  • Provide ongoing supportive care to clients with chronic illnesses and behavioral health challenges, with a focus on engaging underserved and hard-to-reach populations.
  • Facilitate access to early intervention, primary care, and supportive services for HIV-positive individuals, including enrollment and case management for the AIDS Drug Assistance Program (ADAP).
Administration (40%)
  • Supervises HIV Social Services team composed of Medical Case Managers, HIV Patient Navigator, Treatment Link Specialist, and Recruitment and Retention Coordinator to ensure care coordination across disciplines, providing guidance and direction, training, evaluation performance and initiating corrective action as needed.
  • Member of the HIV services Senior Managers Committee, Quality Management Committee and Clinical Operations Committee.
  • Coordinates and facilitates Social Services team meetings and psychosocial case presentations.
  • Reviews new HIV positive cases with Recruitment and Retention Coordinator and assigns to appropriate case manager.
  • Conducts Quality Assurance Activities for the psychosocial services, such as regular chart audits, DPH data entry monitoring, and coordination of regular patient acuity assessments.
  • Schedules monthly in-service trainings with assistance of the HIV Services Program Assistant aimed to improve staff HIV knowledge and skills.
  • Actively participate in weekly Multi-disciplinary Team Meetings; monthly HIV Management meetings; monthly Mission Center of Excellence meeting; monthly Clinical Quality Management Program meetings; monthly Managers and Supervisors meetings; bimonthly MNHC wide General Staff meetings.
  • Work in conjunction with the HIV Services Director to monitor and ensure that grant obligations are met and implement needed strategies to achieve deliverables.
  • Attend weekly multi-disciplinary team meetings and present clients' psychosocial assessment and treatment plan based on recommendations from physicians, health educator, nutritionist, and other team members. Participate in staff meetings and in-service training. Receive weekly individual clinical supervision.
  • Documentation of interventions in clients' records.
  • Develop and implement workflows and ensure medical case managers and social services staff are trained and equipped to perform their duties.
  • Work closely with clinic staff to ensure the HIV services work is aligned with the Health Center’s strategic plan goals and objectives.
  • Conduct performance evaluations, provide feedback and coaching, and manage employee relations, and ensure the retention of social services staff.
  • Work closely with the HIV services medical staff by coordinating the work of the social services team to meet patient needs and troubleshoot access issues affecting patient care. 
  • Monitor the utilization and productivity of medical case management staff, ensure enhanced care management goals are achieved, and set department expectations.
  • Participate and lead quality management efforts focused on department health metrics and oversee the medical case managers’ peer review process.
  • Develop reports and statistical data as required by funding sources and support HRSA and San Francisco Department of Public Health audits.
  • Provide guidance and supports team members to address challenges they may face in the execution of their duties.
  • Ensure compliance with MNHC policies and regulations.
  • Other duties as assigned.
Qualifications and Skills
  • Master of Social Work Degree.
  • Bilingual (English/Spanish).
  • Minimum of two years supervisory experience.
  • Experience working with Latinx population.
  • Experience working in Primary Care setting preferred.
  • Flexibility in scheduling and evening work is required.
  • Ability to work professionally and ethically within multi-cultural and multi-disciplinary settings, including ability to maintain confidentiality and privacy of people, documents, data, and communications.
  • Sensitivity to HIV/AIDS and Gay, Lesbian, Bisexual, Gender Variant, Non-Binary and Transgender issues.

Physical Demands:
While performing the duties of this job, the employee is required to sit approximately 80% of the time, walk 10% of the time, and stand 10% of the time. The employee is frequently required to use hands and fingers to type or handle, feel, and operate objects. The employee is required to occasionally lift and/or move up to 20 pounds. While performing the duties of this job, the employee is occasionally required to reach with hands and arms. The employee is frequently required to speak and listen. Specific vision abilities of this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
