Registered Dietitian (bilingual)
The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services.

We are currently looking for qualified candidates for our Registered Dietitian role. This position reports to the Wellness Director and is a Regular, Full Time, Exempt role with a starting annual range of $75,400 to $83,000 with full benefits. Opportunity for remote work after 60 days of employment.
Primary Objective:
The Registered Dietitian is a critical care team member responsible for assessing the nutritional status of patients, tracking and following-up with patients with chronic diseases, and maintaining a panel of Adult Medicine patients enrolled in Diabetes Care Management. This role closely collaborates with members of the Adult Medicine Care Teams.
Essential Functions/Responsibilities:
- Obtains, records, analyzes and evaluates nutrition and dietary history of patients.
- Develops a patient-centered plan of nutrition plan. Includes dietary consultation to patients and their families on the selection and preparation of food taking into account income, cultural patterns, etc.
- Provides on-going evaluations of patients’ nutritional status and follows up at regular intervals. Participates in Care Team meetings.
- Run monthly panel management reports searches on chronic care conditions and enroll patients into Diabetic Care Management program.
- Tracks and follows up with patients enrolled in Diabetic Care Management.
- Refers patients to food programs such as WIC, CalFresh, food pantries and serves as liaison with these programs.
- Provides self-management education to gestational diabetic patients and monitors their diabetes indicators, as needed.
- Participates in monthly department meetings.
- Participates in Quality Improvement activities.
- Other related duties as assigned.
Qualifications:
- Bilingual in English and Spanish required.
- Registered Dietitian credential.
- Bachelor’s Degree in Nutrition/Dietetics or related field.
- 1-2 years of experience in individual nutritional counseling/group nutrition education preferred.
- Knowledge of diabetes care and diabetes self-management.
- Experience or familiarity with community health centers and/or primary care clinics.
- As a condition of employment, all candidates are required to meet specific immunization standards. Documentation of current immunizations, including Varicella, TDAP, COVID-19, MMR, and Hepatitis B, as well as TB test results valid within the last two (2) years must be submitted before the start date. Failure to provide this documentation may result in a delayed start or withdrawal of the job offer.
