The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.
We are seeking an experienced, bilingual Lead Primary Care Provider—Physician (MD or DO), Nurse Practitioner, or Physician Assistant—to join our team at Mission Neighborhood Resource Center, which serves the unhoused community in the Mission District. The medical clinic consists of 4 exam rooms with 1-2 medical providers working at a time, a dedicated RN, MA's trained in phlebotomy, and part-time support of substance abuse counselor and behavioral health. The clinic is collocated with additional services at the MNRC Drop-in center, which include case management, support groups, shower facilities, lockers, laundry, food support, and referrals for lodging and housing.
PRIMARY OBJECTIVE:
Under the supervision of the Chief Medical Officer, the primary care provider at the Homeless Resource Center delivers preventive and acute care, substance use and mental health counseling, integrated with case management and support services in the Drop-In Center.

PRIMARY OBJECTIVE:
Under the supervision of the Chief Medical Officer, the primary care provider at the Homeless Resource Center delivers preventive and acute care, substance use and mental health counseling, integrated with case management and support services in the Drop-In Center.
RESPONSIBILITIES
- Care for unhoused patients with attention to assessing and addressing social determinants of health
- Care for transgender and gender diverse patients, including managing gender-affirming hormones
- Care for patients with serious psychiatric disease, including managing antipsychotic medications, and for patients who need a higher level of mental health care, bridging and coordinating care with mental health specialists
- Care for patients with substance use disorders, including providing Medication-Assisted Treatment (MAT)
- Treatment and prevention of STIs, including HIV-prevention via PrEP
- Maintain accurate documentation of all services and communications in patient medical records in a timely manner to ensure continuity of care
- Monitor laboratory results and patient communications within the electronic health record promptly and effectively
- Engage in quality improvement initiatives, management activities, continuing education, peer review, and other patient care programs mandated by regulatory, grant, and clinic requirements
- Collaborate with specialists and hospitalists/residents regarding outpatient referrals and patient care plans, adhering to Health Center protocols, and providing continuity of care in transitions between inpatient and outpatient care
- Complete billing encounters promptly
- Advocate for health equity and culturally responsive care
- Participate in organizational leadership meetings and serve as a liaison between RC clinic and organizational leadership (this position is 80% Clinical, 20% Administrative)
- Supervise and provide consultations for other medical providers in the clinic (currently one full-time NP)
- California-licensed MD or DO with Board Certification in Family Medicine or Internal Medicine, Adult/Geriatric NP, Family NP, or PA with adult medicine training
- Fluent in Spanish (required).
- Three years of post-graduate clinical experience in primary care (can include residency programs)
- Experience with underserved populations, including significant work with unhoused populations.
- Physician (MD/DO): $234,600 - 259,590 (FT)
- Nurse Practitioner (FNP): $133,008 – $159,630 (FT)
