Registered Nurse - Disease Management (Bilingual)
The Disease Management RN serves as the clinical lead for care coordination, supporting patients with complex and chronic conditions through comprehensive assessments, care planning, and transitions of care. This role collaborates with an interdisciplinary team to close care gaps, improve outcomes, and connect patients to the resources they need to thrive.
The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We are currently looking for qualified candidates to join MNHC as a Registered Nurse - Disease Management. Please note: This role requires applicants to be bilingual in English and Spanish. This position reports to the Senior Nurse Manager and is Exempt role with a starting hourly range of $47.32 - $50.95 (depending of years of clinical experience post RN license)
Primary Objective:
The Registered Nurse is responsible for the delivery of quality nursing care and will assist in establishing a clinical team approach to comprehensive medical care and provide continuity of care to MNHC patients at multiple locations.
Essential Functions/Responsibilities:
- Coordinates patient flow in clinical area.
- Responsible for obtaining and recording accurate medical information and history. This includes the following:
- Checking patient charts
- Checking encounter forms, prescriptions, lab and X‑ray requests.
- Completing disability, CHPD, and other medically related‑ forms.
- Maintaining tracking systems and follow-up reports/requests.
- Initiating health maintenance and prevention procedures.
- Assesses initial health needs of patients and prepares them as indicated or as condition warrants.
- Provides medical treatment as ordered and performs procedures according to MNHC nursing protocol.
- Prepares and administers medication as ordered.
- Responsible for triage/emergency and medical screening of patients in charge area.
- Provides patient education and counseling.
- Provides telehealth visits, including hospitalization pre and post discharge planning
- Attends and participates in nursing staff meetings as needed.
- Maintains confidentiality of all patient information according to federal guidelines and regulations.

- Valid California Registered Nurse License.
- Bilingual in English and Spanish.
- Two years of experience in ambulatory care clinic or hospital experience.
